William the Conqueror was one of the most famous medieval rulers.  He acquired his title when he invaded and conquered England in 1066.  He was already Duke of Normandy.  In 1087, he had ruled Normandy for 31 years and England for 21.  At 59 years old, he was still putting down revolts.  Even though he was very obese, he insisted in leading his army against the city of Mantes, near Paris.  He took the city, of course.  He never failed.  But upon entering the burning city, his horse reared up, possibly from stepping on a burning piece of wood or from being spooked by some birds.  William was thrust forward and his giant belly hit the pommel of his saddle.  Several internal organs were burst.  He was taken to Rouen and suffered great pain until he died a few weeks later on September 9, 1087.  When he died, all his lords left to safeguard their lands.  Only a few servants were left to prepare the body for burial.  When they tried to stuff the corpulent body into a coffin, the stomach ruptured emitting a horrible smell that cleared the room.  Eventually, a few monks managed to get the great warrior king into the ground.
